Case Management Services at Across Boundaries

We offer free case management services for youth aged 15 to 29 and adults over 29. A mental health diagnosis is not required.

Individuals from Black and racialized communities living in the Greater Toronto Area

You can sign up yourself or get a referral from a doctor or community service provider

How to Enroll

1. Step One

Click on the Access Point form:

2. Step Two

You can fill out the form online, or an intake worker can assist you over the phone or in person.

3. Step Three

The application form includes multiple programs. Applicants should select the program that best meets their needs.

The form also includes a housing application. If supportive housing is needed, applicants can apply for both mental health and housing support on the same form.

4. Step Four

Fill the required information under the sections: contact information, general information, reason for the referral, health information, current supports and agency preferences.

In the “Agency Preferences” section, select “Across Boundaries” in the “Preferred Agency” drop-down menu.

5. Step Five

After you submit the form, Access Point will call to see if you’re eligible. If you are eligible, you will be added to the waitlist.

6. Step Six

We’ll contact you to start services when a spot becomes available.

Psychosocial Rehabilitation ( Day Program)

Our Day Programs offer activities that help with healing and skill-building and promote relaxation. We also provide meals and food hampers (every two weeks), and celebrate different cultures through special events.

Who can join?

People in the Greater Toronto Area who are part of Black and racialized communities and have faced mental health or addiction challenges.

How to join:

Email our Program and Intake representative at lorraine@acrossboundaries.ca or call 416-787-3007, extension 222. We will talk with you first to understand your needs and see if the program is right for you. You can request a monthly program schedule or visit us to try a program before joining.
