Case Management Services

Adult Mental Health Case Management Services

We provide community-based mental health support for racialized and Black individuals aged 15 and older. Our staff work with you to understand your needs, set goals, and build life and job skills—empowering you to be in control of your healing journey.

Our team speaks many languages and comes from the communities we serve. Our comprehensive support includes:

  • One-on-one support 
  • Community outreach
  • Support groups and complementary therapies
  • Community kitchen
  • Skills-building
  • Social & recreational activities

Youth Case Management Services and Programs – Transitional Aged Youth (TAY)

We support transitional-aged youth (ages 15-29) facing mental health and addiction challenges with care and understanding. WW serve youth within the area bordered by Dufferin Street Humber River, Steeles Avenue, and Highway 401.

2SLGBTQ+ Case Management

We recognize 2SLGBTQIA++ communities face unique challenges with housing, education, employment, legal issues, immigration, social services, ODSP and Ontario Works. So, we’re here to help.

Our counsellors –many of whom have gone through similar experiences– are registered with the Ontario College of Social Workers and Social Service Workers, and offer one-on-one support using Cognitive Behavioural Therapy and trauma-informed, strength-based care. We create a safe space by respecting who you are, welcome trans identities, and always put the person first.

At Home/Housing First Case Management Services

At Home helps racialized individuals move from long-term houselessness to living more independently. Based on the Housing First (HF) model, we provide Intensive Case Management to support those facing houselessness and mental health challenges.

Mental Health and Justice Short-Term Community Support

We support racialized individuals with mental health challenges as they move through the criminal justice system. Our staff assists with release planning, court support, and finding alternatives to the criminal justice system. We provide case management services for up to six months to ease the transition out of the justice system and support recovery.

Mental Health and Justice Initiative (MHJI)

We provide mental health and housing support services across Toronto for individuals moving through the criminal justice system. The MHJI is delivered in partnership with the Canadian Mental Health Association.

Psychiatric Consultation Program

Our psychiatrists provide support, advice and learning sessions for staff and service users. We ensure that the care we offer is respectful of all backgrounds and ethnicity. Our support is guided by anti-racism, anti-oppression, and confronting anti-Black racism principles.

The Ontario Structured Psychotherapy (OSP)

OSP provides therapy for depression, stress, PTSD, OCD, and anxiety-related conditions, helping participants build skills to improve their mental health. Guided by Cognitive Behavioural Therapy (CBT), the program helps set goals, is collaborative, and includes light reading and practice.

Services include:

  • Guided Self-Help (The BounceBack Ontario): Phone coaching, skill-building workshops and online videos
  • Clinician-Assisted Bibliotherapy: Weekly 30-minute phone sessions based on a CBT self-help workbook
  • Individual or Group CBT: Weekly one-hour sessions, offered online or in person

Services are available in multiple languages for individuals in York Region, South Simcoe, North York, North Etobicoke, and Malton, and provided through a partnership with the Canadian Mental Health Association (CMHA- York), Across Boundaries, and Hong Fook Mental Health Association.

Psychosocial Rehabilitation (Day) Program

This community-based program helps racialized individuals with mental health and addiction challenges become independent and improve their quality of life. 

We offer workshops and activities to help you learn coping skills, make social connections, and improve your quality of life. Participants can also join yoga, art, and music therapy, and access hot meals, weekly groceries, and nutritional support—because we believe mental and physical health are deeply connected.

Substance Use Health and Wellness Services

Our programs are run by people with real-life experiences, who take a compassionate approach to substance use and addiction. We focus on improving overall health and creating an inclusive environment where service users feel supported, not ashamed.

Short Term Counselling

We offer short-term counselling for individuals aged 15 and older through internal and external referrals. 

 

Family & Caregiver Support

Families play an important role in caregiving, so we’re here to help them too. We provide knowledge, skills, and support to help them care for their loved ones while prioritizing their health. Our families and caregivers’ engagement events combine social connection and support, reminding them they’re not alone.

Acupuncture

Acupuncture involves working with specific points in the body and offers benefits like stress reduction, pain relief, and improved overall well-being.
